LANCO Fieldhouse
WAIVER ASSUMPTION AND RELEASE OF LIABILITY. Turf and contact sports are
inherently dangerous. The undersigned, on behalf of the undersigned and the undersigned's
child (collectively "Participant") hereby: (1) assume the risk of personal injury,
property damage, or other loss (collectively "Injuries") to the Participant arising
from or related to activities at the LANCO Fieldhouse; (2) release Liner-Leeds,
LLC t/d/b/a Lanco Fieldhouse, and its agents, employees, staff members, officers,
directors and members (collectively "LANCO") from all liability, claims, or responsibility
for Injuries to Participant; (3) grant permission for Participant to participate
in activities at Lanco Fieldhouse; and (4) release LANCO from Injury arising from
any good faith acts or omissions in emergency situations. I agree that you may
photograph and/or videotape me or my child during sports activities and that you
retain the right to use these visual images in future literature for LANCO Fieldhouse
without compensation to my child or me. I further agree that you may use my name,
my child's name, or any testimonials made by us without limitation in advertising
and promoting LANCO Fieldhouse. I represent that I am over the age of 18 or a
parent/guardian of the minor named below, and agree that the grant and release
contained therein binds me and the minor of all of its terms. By signing players
and teams agree to be governed by Lanco Fieldhouse rules and polices
- A $50 nonrefundable deposit (or payment in full) per participant for
each session is due with a completed registration form to secure a spot prior
to the session deadline.
- All registrations will be accepted on a first
come first serve basis.
- Clinics and classes cannot be prorated for expected
missed sessions.
- All registrations, money issues, and questions regarding
registrations will be conducted during business office hours.

RULES
How can I register a
team in a league or tournament? - Click on the registration
button located by your desired program.
- Teams must provide a scheduling contact
(coach or team coordinator) and a financial contact (the team's financial manager)
if available.
A nonrefundable deposit
or payment is full is due with a completed registration form is due to secure
a spot. Please see each sport league section for deposit amount.
If you know you will have a conflict with game times, please contact LANCO Fieldhouse
PRIOR to scheduling. Due to limited space, we regret that we will be unable to
reschedule games AFTER the schedule has been distributed. The team that cannot
attend must forfeit the game resulting in a 5-0 loss against that team. Games,
Clinics, Camps, Leagues, Tournaments and in other events that are cancelled due
to incluement weather or for other reasons beyond the control of LFH shall Not
be rescheduled and there shall be no refund or pro-rated refund of any fees or
field rentals. Teams or users who wish to make up the time may, if the schedule
permits, pay an additional rental fee ($75 per rescheduled game) and schedule
new time from LFH. LFH is its sole discretion shall determine if there is availability
to schedule the new time and shall not be obligated to reschedule any team or
user.
LEAGUES & TOURNAMENTS
- All teams will be accepted on a first come first serve basis

registrations, money issues, and questions regarding registrations will be conducted
during business office hours.
What should
I wear and what equipment do I need? - All activities at
the "Fieldhouse" suggest appropriate sports clothes and athletic shoes. Athletic
shoes, turf shoes or rubber-molded cleats can be worn on the turf. Screw-in cleats
are not permitted.
- In addition, these sports require the following equipment/clothing:
- Soccer - Shin guards. All teams must wear t-shirts or jerseys
of the same color with a number on the back.
- Field Hockey: Shin guards
and mouth guard. Don't forget your stick. Goalkeepers bring your gear.
- Boys
Lacrosse: stick, helmet, gloves, elbow pads, shoulder pads, and mouthpiece.
Protective cup is required for all male players. Goalkeepers bring your full outdoor
gear.
- Girls Lacrosse: stick, shin guards, mouth guards, eye goggles.

How do I know if I'm accepted into a program?
You are accepted in the program unless notified by LANCO Fieldhouse staff.
There are no confirmation calls or emails sent. Please call us at 560-0717
if you wish to confirm a regsitration was recieved and processed. Each clinic,
league or tournament has a minimum number of participants and/or teams that must
be registered prior to the registration deadline in order for it to be offered.
If that number is not reached, the program will be cancelled. Any registrations
received for a cancelled program will receive a phone call by LANCO Fieldhouse
canceling the program and full refunds will be issued. If any activity has
reached its maximum limit, a waiting list will be started. Any team or individual
on a waiting list will receive a call from LANCO Fieldhouse. If a team or individual
accepted for participation does not abide by registration and payment policies
or cancels for any reason, teams or individual from the waiting list have priority
by date registration was received.
What is
a Team Form and where can I find one? A Team Form is used to list
the players on your team. To verify the maximum number of players per roster,
please check the rules for your sport on our website. Only those players listed
on your Team Form are permitted to play. Be sure to compete and sign the form
before submitting it. You must be 18 or older to sign the form. Any player under
the age of 18 must have a parent/guardian sign the form. Although this form must
be handed in prior to play, league teams can add or drop players up to the beginning
of their third game, simply prepare a supplemental Team Form to be added to the
form on file. Copies of the form are available at the "Fieldhouse" and can be
downloaded from our website.
BALANCES DUE
CLINICS - Expect confirmation and balance notification by phone or
email after registration deadline in your particular program. Balances must be
paid in full prior to the session start date. LEAGUES - All teams registering
for leagues are required to submit a completed Team Form along with payment in
full prior to the start of the league. We ask that each team submit one check
or credit card payment. Balances are due BEFORE the first game, or the team will
not be allowed to play. If paid in full a week prior to the league start date,
a discount rate will apply (see each sport section for further details).
TOURNAMENTS - All teams registering for tournaments are required to submit
a completed Team Form along with payment in full prior to the tournament. We ask
that each team submit one check or credit card payment. TEAMS WILL
NOT BE PERMITTED TO PLAY UNTIL SUBMITTING A COMPLETED TEAM FORM AND LEAGUE OR
TOURNAMENT FEES ARE PAID IN FULL. A WAITING LIST TEAM WILL STEP IN IF DEADLINE
REQUIREMENTS ARE NOT FULFILLED
CANCELLATIONS
In the event of inclement weather, cancellations will be posted on the
front page of our website at www.lancofieldhouse.com and also on our answering
machine. Please check it!! If we cancel a league or tournament, the team scheduling
contact of each team will be notified by phone. Please call your team contact

there. Also, you may call or email us and put your name on an individual list
and your name and phone number will be given out IF coach's/teams need extra players.
There are no guarantees that you will be placed on a team.
